Screening Methods in Patients With Down Syndrome for Obstructive Sleep Apnea

Introdução

Obstructive sleep apnea (OSA) is a highly prevalent breathing disorder during sleep, which may affect from 1 to 5% of the pediatric population. Down Syndrome (DS) children are considered to be at higher risk

Objetivo

To evaluate the sensitivity, specificity, positive and negative predictive value and accuracy of pediatric sleep screening questionnaires for OSA diagnosis.

Métodos

DS children and adolescents, aged 3-15 years, were invited to answer the screening questionniares OSA 18 and Pediatric Sleep Questionnaire (PSQ), and underwent a fullnight polysomnnography type III. at the Unesp Sleep Laboratory

Resultados

15 children were enrollled. The prevalence of OSA in the population was 93.3%. The PSQ showed a sensitivity (S) of 71.4%, specificity (E) of 100%, positive predictive value (PPV) of 100% and negative predictive value (NPV) of 100%, and an accuracy of 73.3%.. For OSA-18, the results showed S=50%, E=100%, VPN=100, VPN=100, and accuracy of 53.3%.

Conclusões

In DS children, it is essential to investigate sleep disorders, screening tools as PSQ or even OSA 18 being good predictors OSA, even if the parents do not have acute sleep complaints. However, polysomnography is still highly recommended for precise diagnosis. ostic tools and established pediatric scales
