Cloud-based telemonitoring as a strategy to improve CPAP adherence – preliminary data

Introdução

CPAP (continuous positive airway pressure) is the gold standard treatment for patients with moderate to severe obstructive sleep apnea (OSA). However, adherence to treatment remains a major challenge. CPAP adherence varies between 40 to 85% in the first month of therapy and it can be a predictor of long-term use.

Objetivo

To describe CPAP adherence in the first month of treatment after in-home type 3 polysomnography and automatic CPAP titration, when indicated.

Métodos

We conducted an observational, prospective cohort study with patients aged > 18 years with moderate/severe OSA. A type 3 polysomnography (Apnea Link AIR™) was performed and patients with moderate/severe OSA underwent in-home CPAP titration with an automatic device (AirSense 10™) with coupled oximetry for 03 consecutive nights, with daily telemonitoring by AirView™. We excluded pregnant women, those with predominantly central sleep apnea, as well as decompensated heart failure. After titration, patients were referred to acquire CPAP (AirSense 10) and then registered in AirView™ for daily telemonitoring, performed by an experienced professional, responsible for evaluating the data and performing adjustments in CPAP pressures, when necessary. A satisfactory adherence was considered in patients with > 4 hours/night of CPAP use in 70% of the nights evaluated.

Resultados

A total of 53 patients were included in the study until now and 49 completed 1 month of treatment. The mean adherence was 81.8%.

Conclusões

These preliminary data reinforce the importance of daily monitoring of patients treated with CPAP to ensure adherence.
