Clinical Case Report: Use of the jaw protrusion appliance in the treatment of Obstructive Sleep Apnea

Introdução

This presentation of a clinical case report, of a patient with obstructive sleep apnea (OSAS), and the use of a mandible protrusion device, aims to show, through evidential data from polysomnography systems, and the Biologix monitoring night system, the effectiveness of the protrusion device of the jaw. This analysis is performed taking into account the oxygen desaturation indexes, the frequency of desaturations, the minimum desaturation, and the time that the patient remained with saturation below 90%, through a monitoring system during the sleep of that patient.

Objetivo

To assess through evidential data from polysomnography systems, and Biologix monitoring system, the effectiveness of the jaw protrusion apparatus.

Métodos

Patient CLO, 37 years old, male, sought care at a dental office on October 13, 2019, with a polysomnography, with a diagnosis of obstructive sleep apnea. After studying the case, the need for some treatment became evident, and for this, we indicated the mandible protrusion device, which the patient began using at bedtime, starting this treatment on November 1, 2019. Were done tests with BIOLOGIX night monitoring system, to prove the effectiveness of the protrusion appliance on November 12, 2019, and November 26, 2019.

Resultados

Patient with obstructive sleep apnea, confirmed in initial polysomnography,dated in September 3, 2019, with result 558 oxygen desaturations (minimum 76%). Exam compatible with severe sleep apnea. The patient start using the mandible protrusion appliance on November 1, 2019, wich was re-evaluated on November 12,2019 reporting improvement in sleep. In order to have supporting data, we asked the patient to perform a nightly assessment with a monitoring systenm, BIOLOGIX, at home, in this day. As a result, 109 oxygen desaturations ( 85%), and the time with saturation below 90%, was 4 minutes. Test compatible with moderate sleep apnea. On november 26, 2019, 52 oxygen desaturations were observed (minimun 81%), and in just 1,5 min the saturation was below 90%. Test compatible with mild sleep apnea.

Conclusões

The jaw protrusion appliance is an important tool in the treatment of patients with obstructive sleep apnea. It helps to improve the quality of life of patients, as a restful sleep helps in their well-being, both physical and mental.
