Overnight Digital Monitoring for OSA patients treated with Mandibular Advancement Device in the COVID-19 pandemic: An observational study.
Introdução
Polysomnography (PSG) is a laboratory sleep study to diagnose and monitor patients with Obstructive Sleep Apnea (OSA) treated with Positive Air Pressure (PAP) or Mandibular Advancement Device (MAD). However, the PSG was challenging to access during the Covid-19 pandemic. Home sleep test type IV PSG methods are proposed, such as BIOLOGIX, an Overnight Digital Monitoring (ODM), which provides diagnostic performance in detecting OSA. Apnea Hypopnea index and Oxygen desaturation index are the two parameters used in these PSG studies. However, in a recent validation study of ODM, the correlation of both parameters was detected. In Dental Sleep Medicine, the ODM appears to be a valuable tool for MAD therapy follow-up.
Objetivo
This observational study aims to compare and correlate PSG types 1 and 4 in OSA patients’ follow-up with MAD treatment during the COVID-19 pandemic.
Métodos
Clinical records of 35 OSA patients diagnosed with PSG type 1, treated with DIORS MAD, and monitored with BIOLOGIX (basal and with MAD) during the COVID-19 pandemic. All patients were indicated to PSG type 1 with MAD, but just 9 patients were able to do it. Then, we just used the basal PSG and ODM (basal and with MAD) in this study. We used quantitative methods to evaluate the correlation between both PSG data by constructing Pearson and Spearman correlation indices. We used contingency tables according to the numerical data normality evaluation, chi-square tests, and the Cramer coefficient V for categorical data. We used the SAS system's support for data analysis, and the significance level for the statistical tests was 5%.
Resultados
This study included 35 OSA patients (men 59.46%; age:47+/-11years; BMI:26.65+/- 3.24kg/m2; PSG-AHI:14.40+/-14.87/h and ODM-ODI:7.87+/-6.22). We controlled OSA with DIORS MAD and monitored with ODM BIOLOGIX (ODI:3.85+/-4.14). We found Statistical significance in ODI and AHI comparison of SpO2, ODI e AHI mean (standard deviation), and Student t statistic for pared data observed by PSG type 1 and 4 parameters. Also, there was a statistically significant correlation between Heart Rate and ODI with Arousal Index and AHI.
Conclusões
ODM BIOLOGIX is a good tool for follow-up OSA patients in Dental Sleep Medicine practices, mainly during the covid-19 pandemic, with statistical significance in AHI and ODI.
