PSYCHOLOGICAL AND SLEEP ASPECTS IN THE PREOPERATIVE PREPARATION OF OBESE PATIENTS APPLIED TO BARIATRIC SURGERY

Introdução

Obese patients have high prevalence of sleep disorders and also high rates psychologic issues. Few papers correlate sleep quality, somnolence in selected obese and bariatric surgery.

Objetivo

Evaluate risk of anxiety and depression in obese patients and data related today time somnolence regarding sleep as well as sleep quality of these during the pre-operative state.

Métodos

Evaluation starting with 3 (three) questionnaires (Pittsburgh Sleep Quality Index, Epworth sleepiness scale, anxiety and depression scale), reading of medical record, BMI above 35 kg/m2, age between 18-70.

Resultados

Data of 45 obese patients was analyzed, 89% female and only 11% male. The average BMI score is 43,10 kg/m2, being 67% classified with degree II of obesity, while the rest of the group presented obese degree III. When analyzing the results of the questionnaires presented, 58% report some psychological disorder, anxiety and/or depression, 49% of the of the patients interviewed were scored to be classified as anxious, 44% as depressive. In relation to sleep quality, 83% of the patients present low sleep quality or some pathological sleep disorder. When analyzing and classifying the patients with daytime somnolence, 49% of these present positive levels for this criteria. There is a positive correlation between anxiety and sleep quality (< 0.0001), however when comparing anxiety with daytime somnolence, there is no correlation (p = 0.22). When analyzing the indexes of depression and sleep quality also have correlation (p < 0.0001), similarly when comparing the depression scale to daytime somnolence, indicating that depression patients suffering from sleep disorder as well as daytime somnolence (p = 0.0445).

Conclusões

Among the patients in pre-operative bariatric surgery, about half of these havepsychological disorders. In anxious patients there is a relationship between this and sleep quality, without daytime somnolence present. In depression patients,there is a correlation between somnolence as well as sleep quality.
